Several Safety Systems Didn’t Work At Bethesda Gardens

TERRE HAUTE, Ind.  – More information has been released in the death of an elderly woman at a local nursing facility.

75-year-old Veronica Hofman was discovered outside in the yard area of Bethesda Gardens on January 26th.

Autopsy results show she died of hypothermia.

Her death was ruled accidental.

Hofman lived in the memory care unit and suffered from dementia and Alzheimer’s disease.

Authorities say that the courtyard alarms were inoperative and the volumes on the door alarms were turned down.

Those problems have reportedly been corrected.

The surveillance cameras were also not functioning due to construction.
